The Saphira philosophy
The Saphira philosophy in treating skin problems and maintaining healthy and youthful looking skin is to use a customised approach in selecting treatments, procedures and home care regimes. We recognise that there is not a ‘one solution fits all’ in treating skin conditions. Some of the many factors involved in making a customised plan for treatment include the severity of the condition, the skin type, lifestyle commitments, outcome expectations, and of course financial considerations.
The one thing that you can be assured of at a Saphira Clinic is that I will NEVER offer a treatment or product that does not have a proven scientific outcome for a skin condition.

Loose and Sagging Skin
The truth about sagging skin
Collagen depletion in the deep dermis, or the ‘scaffolding’ part, of our skin structure is responsible for the loss of volume, slackness and sagging appearance that becomes evident as we get older. Dermal Filling Treatments are used to temporarily correct facial lines and volumise specific areas of the face. Certain Anti Wrinkle Injections are used to temporarily soften muscular lines and creasing of the face, usually around eyes and forehead. Tissue stimulators like Thermage, Injectable Tissue Regeneration and Specialised Platelet Rich Plasma, over time, usually between 6 to 12 months, encourage cell development and increase the collagen in the skin, naturally volumising the treatment area.
Don’t want an artificial look?
We’ve all seen that over filled, frozen or distorted look, often the result of an overuse or incorrect placement of temporary fillers, wrinkle injections, surgery or laser resurfacing. Tissue stimulators like Thermage, Injectable Tissue Regeneration and Specialised Platelet Rich Plasma offer a natural look and a more long term result for the treatment of loose and sagging skin.
The best treatments for loose and sagging skin
Thermage for loose and sagging skin
Thermage uses focussed radio waves to stimulate your body to increase the production of your own collagen, creating a subtle increase in facial volume and a contraction of elastin fibres resulting in skin tightening. This once only treatment uses radio waves to heat the Deeper Dermis (the scaffolding) causing collagen contraction. This collagen stimulation continues over a 6 to 12 month period resulting in a natural lifting and tightening of the skin, while creating uniform volume over the treatment area. Treatment results last anywhere from 2 to 3 years.
PRP for loose and sagging skin
PRP (platelet rich plasma), sometimes referred to as the “Vampire Facelift” or the “Dracula facelift”, is a breakthrough in filler and skin volumising technology using your own DNA and blood to create volume in the skin’s tissue.
Platelet Rich Plasma treatment poses no risk of allergic reaction. Saphira Specialised Platelet Rich Plasma uses the platelets from your own blood cells to restore volume to the skin by producing increased cell development underneath the skin, decreasing lines and folds in the skin.

By injecting the isolated platelets into the dermis of the skin, the recognition of this will stimulate a wound healing response. This wound healing response will initiate new collagen, which improves texture and elasticity of skin. Extra benefits include cell renewal, increased blood flow and oxygen supply, improvement of pigmentation and rapid healing of wounds and volumising and firming slackened skin.
Injections for loose and sagging skin
The Saphira Injectable Tissue Regeneration procedure uses a sterile suspension of Poly-L-Lactic acid, from the fruit acid family. It is biocompatible to human skin tissue and is easily and safely broken down or metabolised by the body. The product has been used since 1999 in more than 150,000 patients, in more than 30 countries, primarily for cosmetic use. In Australia it is TGA approved for aesthetic medicine and other clinical medical applications of facial reconstruction and volumisation.

When the product is injected into the deep layers of the skin, it stimulates the body to gradually rebuild firmer and more resilient collagen around the placed product, creating a unique framing effect. The facial frame becomes thicker, more elastic and tighter.
Acne
Acne has various grades of severity. It is thought that up to 80% of the population suffers from some form of acne at some stage in their lives. Acne can be anything from ‘blackheads’ (closed comedones), ‘whiteheads’ (milia), ‘pus-filled pimples’ (pustules), ‘hard sore lumps’ (papules), to cysts and nodules. The milder forms of acne are manageable with topical treatments and skin care, while more severe acne may require treatment with medication.
The best treatments for acne
Mild acne is treated with a customised combination of therapies like Bionic Skin Polishing, Saphira Anti-ageing Technology Facials and Multi-level Photodynamic Therapy. In addition to a selection of these therapies, acne scarring can be treated with Clear + Brilliant Fractional Laser, Fraxel Laser, CO2 Fractional Laser, Collagen Induction Therapy and E-Light Photo Rejuvenation.
Saphira Skin Care Systems, containing scientific levels of cosmeceutical ingredients, assist acne management on a daily basis.
The best treatments for pigmentation
Excellent results can be achieved on sun-induced skin discolouration using devices like E-Light Photo Rejuvenation, Clear + Brilliant Fractional Laser, Fraxel Laser, CO2 Fractional Laser and Technology Clinical Skin Peels along with melanin suppressing skin care ingredients from the Saphira Skin Care Systems range.
Three types of pigmentation
- Epidermal (superficial) pigmentation – located close to the skin’s surface and caused mainly by sun exposure.
- Dermal pigmentation – found in the dermal layer of skin.
- Combination dermal/epiderma pigmentation – penetrate through both the deeper layers of skin as well as the superficial layers. One example of this is Melasma.
Melasma treatments
Saphira have devised a safe and effective management program to help treat Melasma, which does not include the use of IPL or laser devices. Clinical research indicates that after using these devices, there may be an initial improvement in colour regulation, but that the heat generated from the use of these devices can trigger the melanocytes into producing more pigmented areas and create a more adverse outcome. We are concerned about the long term management for our clients in the control of Melasma and our unique customised programs offer clients more options for ongoing, clear skin confidence.
Skin Discolouration and Pigmentation
Skin pigmentation refers to spots that usually occur in sun exposed areas on the body. Skin pigmentation is also known as age spots or liver spots. The increased pigmentation of age spots is brought on by ageing, over exposure to the sun or other forms of ultraviolet lights.
Sunlight, environmental factors and the natural passage of time all contribute to the development of dark spots, skin pigmentation and freckles. Your face, hands and chest are the first parts of the body to show signs of ageing because they receive so much sun exposure. Age spots, skin pigmentation and freckles are forms of pigmented lesions. They are caused by excess production of melanin in the skin by cells called melanocytes.
Skin pigmentation can also occur in pregnant women due to the dramatic hormonal change the body undergoes. In some cases the skin pigmentation in pregnant women disappears after child birth. Alternatively, it can have the reverse effect and worsen.
We’ve all seen that over filled, frozen or distorted look, often the result of an overuse or incorrect placement of temporary fillers, wrinkle injections, surgery or laser resurfacing. Tissue stimulators like Thermage, Injectable tissue Regeneration and Specialised Platelet Rich Plasma therapy offer a natural look and a more long term result for the treatment of loose and sagging skin.
Thermage for treating lines and wrinkles
Thermage stimulates your body to increase the production of your own collagen creating a subtle increase in facial volume and a contraction of elastin fibres resulting in skin tightening. The once only treatment uses radio waves to heat the Deeper Dermis (the scaffolding) causing collagen contraction. This collagen stimulation continues over a 6 to 12 month period resulting in a natural lifting and tightening of the skin, while creating uniform volume over the treatment area. Treatment results last anywhere from 2 to 3 years.
Injectable tissue regeneration for lines and wrinkles
A safe and biodegradable product is injected into the deep layers of the skin encouraging the body to produce a more firm and resilient collagen frame around the product. As this collagen gradually builds the product biodegrades leaving the increased collagen creating a deep framework or (scaffolding) for the upper facial tissues, creating volume and a tightening of the skin.
Lines and wrinkles
Wrinkles, also known as rhytides, can be both dynamic and static. Dynamic wrinkles are those that are produced by the movement of muscles in the face, forming what is commonly known as ‘expression lines’. Static wrinkles or wrinkles-at-rest, are those that are visible without facial muscle contractions, these creases may have initially been caused by muscle contraction but are now persistent.
The best treatment for lines and wrinkles
Anti-Wrinkle Injections (neuromodulator) are most beneficial in treating dynamic wrinkles, and Dermal Filling Treatments are most helpful for static wrinkles.Treatments such as Multi-level Photodynamic Therapy, Clear + Brilliant Fractional Laser, Fraxel Laser, CO2 Fractional Laser and Collagen Induction Therapy can increase collagen synthesis in the skin and bolster the underlying support fibre of the wrinkled areas to slow deepening and refine appearance.

Treatments such as Anti-Ageing Facial Treatment, Technology Clinical Skin Peels, Clear + Brilliant Fractional Laser and Fraxel Laser and CO2 Fractional Laser can prevent the build-up of dead skin cells on the surface of the skin which exacerbate the appearance of wrinkles. Furthermore, the regular use of clinical skin care can increase the synthesis of collagen and normalise skin cell function.
Fractional Laser for treating lines and wrinkles
The use non-ablative lasers like Clear + Brilliant Fractional Laser, Fraxel Laser and CO2 Fractional Laser, where the laser causes hundreds of “pin holes” in the skin offers the opportunity for the skin to repair and regenerated itself. The healing process promotes new, healthy skin to re-grow underneath the outer lasers of skin. This also helps recreate more volume in the skin, making lines less indented, creating a smoother surface. Fractional Laser treatments treat about 10% to 50% of the surface area of the skin, so multiple treatments are necessary for getting the best result.
Specialised Platelet Rich Plasma for lines and wrinkles
PRP sometimes referred to as the “Vampire Facelift” or the “Dracula facelift” is a breakthrough in filler or skin volumising technology using your own DNA and blood to create volume in the skin’s tissue.
Platelet Rich Plasma treatment poses no risk of allergic reaction. Saphira Specialised Platelet Rich Plasma uses the platelets from your own blood cells to restore volume to scars by producing increased cell development underneath the scar depression.

Exfoliation and regeneration helps reduce uneven skin colour and smooths and refines the surface of the skin, creating a healthy luminosity appearance to the skin.
Cellulite
What causes cellulite?
Cellulite is thought to occur in 80-90% of post-adolescent females. There appears to be a hormonal component to its presentation. It is rarely seen in males. The causes of cellulite include changes in metabolism, physiology, dieting too hard or too much, sex-specific dimorphic skin architecture, alteration of connective tissue structure, hormonal factors, genetic factors, the microcirculatory system, the extracellular matrix, and subtle inflammatory alterations, most of which are out of your control.

Hormones play a dominant role in the formation of cellulite. Estrogen may be the important hormone to initiate and aggravate cellulite. However, there has been no reliable clinical evidence to support such a claim. Other hormones, including insulin and the thyroid hormones, are all believed to participate in the development of cellulite.
There is a genetic element in individual susceptibility to cellulite. Several factors have been shown to affect the development of cellulite. Sex, race, biotype, distribution of subcutaneous fat, and predisposition to lymphatic and circulatory insufficiency have all been shown to contribute to cellulite.
The best cellulite treatments
Saphira Thermage technology for skin tightening and cellulite removal uses mono-polar radio frequency. The treatment area is heated to a therapeutic temperature range. A number of passes are performed over the area, maintaining an optimum and predetermined temperature range. This process commences the breakdown and removal of unwanted fat and cellulite from specific areas of the body and contracts elastin fibres, strengthening and firming the treatment area.

Stretch marks
What Causes Stretch Marks?
Stretch marks are a form of scarring, caused by the tearing of the dermis, often the result of the rapid stretching of the skin associated with rapid growth or rapid weight changes. Stretch marks may also be influenced by hormonal changes associated with puberty, pregnancy, bodybuilding, hormone replacement therapy, etc.
Stretch marks are often formed during pregnancy, usually during the last trimester, and usually on the belly, but also commonly occurring on the breasts, thighs, hips, lower back and buttocks.

Genetic factors may also play a role.
Basically, if your mother had them, you’re probably going to have them. While it is crucial to moisturise the area where skin is likely to be stretching or dramatically increasing in size, there isn’t any scientifically proven prevention treatment for stopping you from getting stretch marks. Stretch marks don’t look the same on everyone. The appearance of stretch marks depends on the colour of your skin; they can start out pink, reddish brown, brown, or dark brown, and fade over time to a more silvery colour.
The best treatments for stretch marks
When stretch marks show up, it’s best to treat them as early as possible.
Research has focused on the early stages of stretch marks, when they are still red or purple and most readily respond to treatment. Once stretch marks pass the initial stage, when they are red or purple, to the later stages, where they become white or silver – often with deep indentations – they are much more challenging to treat.
During pregnancy, experts recommend moisturizing three or four times a day with products that contain cocoa butter or shea butter as a prime ingredient. Massage the moisturiser deep into breasts, belly, hips, and buttocks.
Many products claim to “repair” stretch marks. But only a few work.
Here’s a rundown of the products available for stretch mark removal and what they can – and cannot – do:
- Wheat germ oil: There is not much scientific data on whether home remedies for stretch marks, such as wheat germ oil, can help. One recent study did find it helped improve stretch marks in their early phase.
- Bio-Oil: Again during pregnancy and after pregnancy, a moisturiser such as this can help with moisturising the area making it more plasticised, but there are no ‘magic ingredients’ in this product to prevent you from getting stretch marks.
- AHAs (Alpha Hydroxy Acids): AHAs work on stretch marks by boosting collagen production. AHAs are found in the Saphira Skin Care Systems and in the Technology Clinical Peel Treatments, which are administered in higher professional doses as peel treatments to help increase these effects.
- Vitamin C: The application of concentrated vitamin C found in Saphira Skin Care Systems, in conjunction with professional treatment of Collagen Induction Therapy, increases collagen production and helps early-stage stretch marks. For maximum effect, we combine with AHA treatments and fractional laser. Vitamin C supplements may also be effective. Experts suggest 500 milligrams three times a day.
- Peptide-containing products: Peptide-containing products are widely marketed as “repair” creams and are a waste of time and money according to experts like dermatologist Dr Leslie Baumann who says “Despite commercial claims, there is no convincing data that these work.”
- Retinoids: Retinoids have been shown to be effective in increasing collagen and elastic production during the early stages. But you should avoid them if you’re pregnant or nursing. If you are nursing you could use preparations in the form of Retinol in up to 1% concentrate from the Saphira Skin Care Systems.
- AHA’s plus retinoids: Using these together may provide better results. But again, retinoids are not to be used when you’re pregnant or nursing.
Derma Roller or Derma Pen Collagen Induction Therapy for treating stretch marks
This can be effective once the red/purple colour stage of the stretchmarks has been treated with selected filter with the E-Light Photo Rejuvenation device and the stretch marks are white in colour. The derma Pen or skin roller, or Collagen Induction Therapy as it is referred to, is an instrument consisting of a heavy duty plastic roller head covered in high quality titanium needles. The needles are designed to penetrate the stratum corneum, the hard outer surface of the epidermal layer of the body skin. Hundreds of tiny channels are created through the stratum corneum, which facilitate the passage and absorption of ingredients like AHAs and Vitamins A & C. In addition the rolling action helps to break up the scar tissue of the stretch mark allowing new collagen formation resulting in a more refined and less intense groove.
Fractional Laser for treating stretch marks
This popular treatment option is used by many skin treatment professionals and they are also being tried on stretch marks, as well.
“Lasers promote synthesis of healthy, new collagen, which has been damaged when stretch marks appear,” says New York dermatologist Linda K. Franks, MD. She uses lasers at her practice to treat red/purple and white stretch marks.
For red and purple marks, Franks uses a vascular laser or IPL photo rejuvenation that targets swollen and inflamed blood vessels and helps with skin cell production and increased collagen production. “Vascular lasers and IPL photo rejuvenation devices won’t take away the superficial skin but will take away the redness,” she says.

Lasers that help to minimize older, more entrenched stretch marks are the fractionated lasers – Clear + Brilliant Fractional Laser, Fraxel Laser or CO2 Fractional Laser – which hits tiny “fractions” of the skin, often in a grid-like pattern. Franks describes the process as “smudging” the lines of stretch marks, which makes them less distinct.
Just don’t expect perfection.
“People are resigned to the fact that stretch marks are permanent and can’t be fixed, but there are ways to treat them.” Franks says. “There will always be some left, though, whether you’re treating the red ones or the white ones.”

What causes skin to scar?
One of the main components of skin is a protein called collagen. Collagen is primarily made from specialized cells called fibroblasts. After scab formation, fibroblasts that are hanging out under the surface of the skin will begin leaking into the clot just under the scab. Once the clot is sufficiently soaked with fibroblasts, these cells begin to secrete collagen and release a growth factor. The growth factor allows the fibroblasts to begin expressing their contractile proteins. This changes them from migratory cells into a cell that can contract and pull a wound tightly together.

Even though scar tissue is made from the same type of proteins as normal skin, the difference in appearance stems from the composition of the proteins. Specifically, instead of lying in a “basket weave” type structure, the proteins are pulled into alignment in one direction. Scars also do not contain sweat glands, hair follicles, or skin protecting sebaceous glands. Because of this, their texture is usually smoother and can become very itchy.
Fractional laser for scars
A long term solution in the treatment of acne scarring is to use non-ablative lasers like Clear + Brilliant Fraction Laser, Fraxel Laser and CO2 Fractional Laser, where the laser causes hundreds of “pin holes” in the skin. The healing process promotes new, healthy skin to re-grow underneath the scar.

This also helps recreate more volume in the skin, making the scar less indented, creating a smoother surface. Multiple treatments are of Fractional Laser are necessary for getting the best result. We recommend a minimum of 6 treatments of any of the fractional lasers depending on the severity of the scarring.
The best treatment for scars
The treatment selection for scars depends on the type of scarring. Typically, when a client presents with acne scarring, we treat each deep or pitted scar individually as required, and then consider Clear + Brilliant Fractional Laser, Fraxel or CO2 Fractional Laser for the affected areas. The treatments of each individual scar depends on the type of scar and other features of the scar including width and depth of the scar, and what the edges are like.

When clients want an instant volumising of an isolated scar we usually treat by lifting the scar with a dermal filling product, especially hyaluronic acids, which are the base of most dermal filling products on the market today. Clients LOVE this treatment as it only takes about 30 minutes and gives an obvious, instantaneous result. This temporary solution can be offered in conjunction with a treatment where a more long term solution is sought with a treatment like Specialised Platelet Rich Plasma.

Acne scars get worse with ageing
One factor that is often overlooked in the management of acne scarring, is that acne scarring often gets worse with ageing. The main reason for this is due to volume loss, so as the patient ages, it is extremely important to keep on top of their facial volume. Tissue stimulators like Thermage, Injectable Tissue Regeneration and Specialised Platelet Rich Plasma and CO2 Fractional Laser are recommended to maintain facial volume.
